what to expect on your first visit

NEW PATIENTS

Help us get to know you before your first visit – after booking, you’ll receive a link to complete a New Patient form through HotDoc. Please complete this form before attending your appointment.

How to find us

We’re located on the corner of South Pine Road and Woolworths Street in Everton Park, QLD. Look out for the GO2 Health sign – Harmony Early Learning is located directly above us.

You’ll find our reception entrance directly off South Pine Road. We also have undercover parking for patients and visitors, with elevator access to our reception area – you can enter the car park via Woolworths Street.

Waiting for your appointment

  • Check in with our friendly reception team when you arrive.
  • Make yourself comfortable in our waiting area and help yourself to water if needed.
  •  Bathroom facilities are conveniently located next to the waiting area.

Quiet Waiting Area 

We understand that some patients prefer a quieter environment. We offer a dedicated waiting area for people with sensory needs – just let our reception team know when you check in.

What to bring  

Please have these handy for your first appointment with us:  

  • Medicare card 
  • A list of your current medications
  • A valid payment method for any fees or gap payments 
  • A list of questions or concerns to ask your practitioner 

 

We recommend you also bring the following if you have them:  

  • Private health insurance details 
  • Pension or healthcare card 
  • Previous test results/scans 
  • Referral letter (if you have been referred from elsewhere) 
  • Comfortable clothing you can move in (for your physiotherapy or exercise physiology appointment)
